Doctors Struggle With Unpaid Claims From State

Here is an interesting article. I know many of us cannot choose all our patients but whether it be one insurance company or opening an office near a plant you cannot put lots of eggs in one basket. MJ

These are desperate times for some doctors in our state.

Struggling doctors in Oklahoma claim medical services and their own livelihood are on the line, and a state agency is to blame.

A serious backlog of insurance claims is forcing some doctors and dentists into making cutbacks.

Lawmakers said they're on top of the problem, but some providers argue it's too late.

Dr. Steven Watson has thousands of patients, and has run a successful office in the metro for nearly 30 years.

Dr. Watson fears he'll soon be homeless and he may have to close his practice. He can't pay his truck note and his family has little money for food.

"You're completely crumbled and just humbled when a patient comes to your house and brings you meatloaf," Dr. Watson said.

ADA Convenes Major Summit On Improving Access To Dental Care

Nearly 150 stakeholders from non-profit groups, government agencies and private industry met March 23-25 at a summit on access to dental care convened by the American Dental Association (ADA) to create a common vision toward improving the oral health of underserved populations.

"Both the private and public sectors are challenged to meet the needs of an ever-growing number of U.S. residents who cannot regularly access oral health care," said John S. Findley, D.D.S., ADA president. "In many ways, this is a societal issue.

"We are very pleased with the success of the summit, which will serve as a milestone toward our common goal of improving access to dental care. The ADA firmly believes that practical solutions must come from a broad spectrum of stakeholders and then be implemented first among those populations at highest risk for oral disease and with the least ability to access care. We are committed to finding common ground and shared solutions to address the oral health needs of the most vulnerable among us."

Participants included dental special interest groups, federal agencies, health care policymakers, advocacy groups, dental industry, dental education and research communities, financing communities, including third-party payers and philanthropic organizations, safety net providers, non-dental health care providers, ADA leadership, dental volunteer leaders and state dental society executive directors.

The participants shared their knowledge and experience and discussed new and existing approaches to reducing disparities and securing optimal oral health for all. Among the major topics discussed were increasing collaboration between the dental and medical communities; workforce development strategies; strengthening dental delivery systems, the dental public health infrastructure, and population-based prevention strategies; improving oral health literacy through social marketing; and finding better ways to define and measure the access issue.

The organizers' intention is that the summit's success will strengthen existing relationships among dental professionals and other stakeholders and encourage a continuing dialogue.

The meeting was held at the ADA's national headquarters in Chicago and was underwritten primarily through sponsorship from the Wm. Wrigley Jr. Co., with the support of Procter & Gamble and the Dental Trade Alliance.

BIOLASE and NLT Announce Settlement Agreement

IRVINE, CA and INDIANAPOLIS, IN--(Marketwire - March 26, 2009) - BIOLASE Technology, Inc. (NASDAQ: BLTI) and National Laser Technology, Inc. (NLT) announced today that they have reached a final settlement agreement resolving the claims and counterclaims in the lawsuit between NLT and BIOLASE pending in the Southern District of Indiana concerning NLT's sale of used BIOLASE dental lasers. Both parties' claims will be dismissed with prejudice.

NLT had brought the suit in August 2008 alleging, among other things, that BIOLASE made false statements to dentists concerning NLT's sale of used BIOLASE lasers and refused to sell laser tips and other parts to dentists if they did business with NLT in violation of antitrust and unfair competition laws. BIOLASE filed counterclaims alleging that NLT infringed BIOLASE's copyrights and trademarks, made false and misleading statements on its website, and modified, sold, and serviced used BIOLASE lasers without premarket approval from the FDA in violation of unfair competition laws.

While each party continues to deny the allegations against it, the parties have agreed to resolve their dispute. The parties are looking forward to selling their respective products.

DENTISTS in Plymouth are pulling out more than twice the number of teeth sparking renewed concerns over problems accessing NHS treatment.

Tooth extractions among adults in the city have rocketed by a startling 114 per cent in recent years official figures reveal.

Health chiefs say the massive rise is down to many more people now receiving regular check-ups, who may not have been to a dentist for sometime, and so raising the likelihood of needing treatment including extractions.

But government critics blame the significant hike on the "fundamentally flawed new dental contract", claiming it does not provide for adequate preventative care.

An independent inquiry, ordered by ministers, is currently being held into NHS dentistry after fierce criticism of the deal.

The contract was introduced back in 2006 in a bid to improve access, but it then emerged significantly fewer patients across the country were actually using NHS services, compared with when the arrangement started.

It has proved unpopular with the profession, and there has also been criticism that because dentists now receive a flat salary, they no longer have any financial incentive to carry out difficult work such as crowns and bridges.

A think-tank now argues dentists should have a limit on the amount of private work they can do.

The New Local Government Network, which specialises in public service reform, said imposing a quota would help improve access to NHS dentistry. It said such a move was justified as it cost £175,000 to train a dentist.

But dental leaders said they were against the idea of quotas, pointing out that there were dentists who wanted to do more NHS work but could not.

Department of Health figures broken down by Primary Care Trust show that the number of extractions of teeth by dentists in Plymouth leapt from 277 in 2003/04 to 593 in 2007/08. In one year alone the number of extractions carried out leapt by nearly 200 from 409 in 2006/07.

In the neighbouring Devon PCT area, the tooth extraction count soared by a third from 1,475 in 2006/07 to 1,966 last year.

Shadow Health Minister Mike Penning said: "The number of extractions is rising, and concerns will quite rightly be raised about the types of treatment people are receiving.

"Patients deserve more than complacency from the Government, and it is time Ministers admitted their contract has been a failure."

Emily Street, dental lead for NHS Plymouth said "Over the last few years we have been able to place many more people with NHS Dentists in Plymouth.

"In fact since 2006 we have placed more than 28,000 people with a regular dentist. Consequently more people are now receiving regular treatment, some of whom probably have not seen a dentist for sometime meaning they are more likely to need treatment including extractions.

"In Plymouth, anyone who is requiring an NHS dentist can add their name to the NHS Plymouth database which is a list of names of those who wish to access a regular dentistry. Patients should write stating name, address, telephone number and date of birth to: NHS Plymouth, FREEPOST, NAT 22278, Plymouth, PL6 5QZ."

Luke Woolley, primary care manager for Devon PCT, said since the introduction of the new contract NHS dental services had been expanded in the county, leading to a significant increase in the numbers of new patients being seen.

He added: "Some of these patients may not have seen a dentist for sometime and therefore they may be more likely to need treatment which could include an extraction.

Dentist wins first round in Yelp review case

This is a big deal! There are many Internet sites that rate dentists and there is no way to contradict what is written. Facts are one thing defamation is another. MJ

The dentist who sued a reviewer on Yelp.com has won the first round of her court battle. Finding that she has at least minimal evidence of libel, a judge overruled the reviewer's motion to dismiss the suit under a special California statute.

Dental caries in a Portuguese identified skeletal sample from the late 19th and early 20th centuries

Abstract
Dental caries was investigated in 600 adult dentitions belonging to the identified osteological collections of the Museum of Anthropology, University of Coimbra, Portugal (late 19th/early 20th centuries). The main advantage of this sample compared to an archaeological source is the presence of known demographic parameters such as age, sex, and occupation. The aim of this study is to investigate the issues involved in comparing caries data derived from archaeological death assemblages with statistics compiled from clinical studies of the living. When only the upper dentition was considered, higher rates were observed in females than in males. No differences were found between sexes for lower teeth. In both sexes, both the percentage of carious teeth and the severity of lesions were found to increase with age, demonstrating that caries activity continued throughout life. The slight decrease observed for the age group 70-79 years is probably due to the increased antemortem tooth loss in the elderly. Caries was most common at contact areas (32.9%) and rarest at smooth crown surfaces (6.5%). Root surface caries was graphed in relation to the exposure of roots, and it was confirmed that the degree of root exposure was not strongly related to the frequency of carious lesions on the exposed root surface, although both increased with age. Molars were attacked more frequently by caries as a whole than premolars, canines or incisors. Fed up with offices who cannot get me digital xrays

Today my office received another unreadable full mouth series of radiographs printed on copy paper with a laser printer. My office manager called the office and requested digital copies. She was politely told that they could not do this because they do not have Internet access and have no idea how to put the radiographs on a CD or any other media so that they could be sent to us.

This is totally unacceptable in this day and age. Either I have to retake the radiographs (at my cost and submit the patient to more radiation)or try and work with something that is unacceptable. I wish there was a way to force these offices to comply with a request for radiographs. I bet if the state board of dentistry asked for the radiographs a phone call would go out to their tech support person.

So for all the offices that don't know how to transmit radiographs securely online or even burn a CD and mail it, please learn how. It makes your high tech office look incompetent to both my office and your former patient. Call your software tech, trainer or IT person and find out how to do it.

Playing with the Kindle 2. A few thoughts.

So I spent a few minutes with my wife's Kindle 2 this morning. The device was easy to use and easy to read. It connected to Amazon and retrieved information quickly. It felt fine to hold it in my hand.


One thing I did not like is that normally free content on the Web comes with a cost on the Kindle. What's up with that? Blogs and newspapers that are free in my web browser on my PC cost money on the Kindle. Well for me since I don't read many books the Kindle would not be something I would get for myself. I think its just as easy to fire up my Dell Mini 9 and read the free content for free.

Many Americans say they forgo routine dental care

Results out this week from a Gallup-Healthways poll of 355,334 Americans indicate that more than one-third of the population, 34% to be exact, did not visit a dentist last year at all. A recent American Dental Association survey shows dentists' incomes are dropping, too, and some dentists say patients who've lost dental insurance are asking about alternate payment plans or skipping appointments.

The 2008 Gallup data report that in nine states, close to half of residents did not visit a dentist last year, even though the ADA recommends twice-yearly cleanings and check-ups. Men, African Americans, Hispanics and seniors were more likely to skip, an historic pattern, Messina says. Over half of those making less than $2,000 a month reported omitting visits as well, according to Gallup.

Here is a quick implant uncovering case done the other day. A little gingival healing over the screw. A few drops of anesthesia (where you see the small amount of bleeding) and simple to now place the transfer coping for an impression in a bloodless field. I used an Nd:Yag laser with 3 watts.

Something to think about


There were 2 news stories recently about NJ dentists. One was a large malpractice award and another was a story on a dentist being sued for referring a few patients to an oral surgeon who was convicted of groping the patient. So how does this come together?

Would it not be prudent to stop referring to a specialist who now has a bad public image? No matter what the facts are would this influence your decision to refer? I sure don't have the answers to these questions but it does make me think before I pick up the card for my next referral.

It's a promise many parents make before taking their children to the dentist: Don't worry, it won't hurt too much. But at some dental clinics catering to low-income families on Medicaid, that promise was broken behind closed doors.
Parents, ex-employees claim extra procedures, poor treatment at dental clinics.

Sources say because Medicaid pays low reimbursement rates, some clinics push to make a profit by performing more procedures than necessary.

Parents claim that the Medicaid Dental Center clinics in North Carolina were more concerned about profits than they were about patients. As a result, their children were left with awful memories, even though several years have passed since their visits.

Dentists and laboratories can now offer implant patients the natural beauty of shaded Lava zirconia that can be custom-designed for the individual patient. BIOLASE Launches New Waterlase® MD TurboTM All-Tissue Dental Laser System

Newly FDA-Cleared Technology Significantly Upgrades Cutting Speed, Comparable to Traditional High-Speed Drill

Thoughts About 2009 Chicago Mid Winter Meeting

Well I got back from cold Chicago into snowy NJ. We will get about 10 inches of snow before today ends. I am sitting in the office waiting for the day to start and figured this was a great time to blog.
Here is my recap of the 2009 Chicago Mid Winter Meeting.

My first day in Chicago was spent filming a new dental entertainment program. More information will follow as we get closer to its release.

Attendance was down but people were buying products! Thanks to all for keeping the economy going. All the companies were telling me that they were having a good meeting. This is in stark contrast to the last two big dental meetings.

Lots of new composites on the market. Heraeus has Venus Diamond and Kerr has Herculite Ultra. Ray Kurdy has officially launched his KöR Whitening Deep Bleaching™ System (see my previous post for more information).

There were no Holy Cow amazing new products that I saw. Here are a few that have caught my eye.

Skyview is a new 3D CBCT machine from Myray. The cost is $129K.



The ProDrive System is comprised of the ProDrive Replacement Turbine, which installs into your existing handpiece; and the ProDrive Bur, which locks into the turbine with its triangular bur shaft. The triangular shaft means less bur slippage. Hopefully I will get to try this out soon and I will report back.

Guru- the patient eduction software has some new animations coming out this year.




Twisted Files from SybronEndo. I just love these files. They make endo so much easier. One of the limits of these files currently is they only come with a 25 size tip. Well soon you will be able to get larger apexes. The .06's will be in sizes 30 and 35 along with a .04 size 40!

Air Techniques Spectra is still not FDA approved for caries detection but I am hoping it will be released in the very near future. I cannot wait to try it out.

As usual the meeting went by in a blur but it is one of the don't miss meetings in the country. It is worth the trip to Chicago even in the dead of winter. More information on the meeting is on the Chicago Dental Society web site.
